OWNER SURRENDER FORM

We understand there are times when there seems to be no other alternative than to try and rehome your companion. While we focus mostly on dogs that are in high-kill shelters, occasionally we are able to help out in an owner surrender situation. We MUST have the form (link below) filled out completely and records, if available, from current or previous vet. The more info you give us, the more we can try to help match the dog with a new family. We do not guarantee that we will be able to help in all situations. Once we receive the form, we will see if there is someone in your area who could asses the dog and we will contact you.
